يقوم بملء استبانة مبينا فيها اسم الشاكي و عنوانه و موضوع الشكوى
و ان اقتضت الضرورة يطلب من الشاكي تقديم الشكوى كتابيا و دعمها بالحجج اللازمة و يسجل الشكوى ثم يوجهها للجهة المختصة للبت فيها
