نعم أنا العامل/محمد كوتي تانيل تودي هندي الجنسية رقم الاقامة / 2273550778أتعهد بأن كفيلي / مؤسسة معتوق مصلح المولد والذي اعمل لديه بمهنة/ عامل تعبئة وتغليف أبلغني بأنه يجب علي عدم العمل خارج المنزل وان لا أقوم بأي عمل يخصني سوى عملي الذي حضرت من بلدي لأجله وهو العمل لدى كفيلي وفي حال تم القبض علي بأي أعمال خارج نطاق عملي فأن كفيلي لايعلم بها وخارج مسئوليته واتحمل مايترتب عليها من تبعات وجزاءات وعقوبات .
