نعم أنا العامل/ بنجلاديشي الجنسية أتعهد بأن كفيلي المدعو/ والذي اعمل لديه بمهنة/ عامل أبلغني بأنه يجب علي عدم العمل خارج المؤسسه وان لا أقوم بأي عمل يخصني سوى عملي الذي حضرت من بلدي لأجله وهو العمل لدى كفيلي وفي حال تم القبض علي بأي أعمال خارج نطاق عملي فأن كفيلي لايعلم بها وخارج مسئوليته واتحمل مايترتب عليها من تبعات وجزاءات وعقوبات .
كذلك اتعهد بأني تبلغت من كفيلي عن نظام وزارة العمل الجديد وماينص عليه من عقوبات وغرامات مالية وسجن لقاء عمل العامل عند شخص غير كفيله او يمتهن أي عمل ليس الذي حضر له من بلده وعلى ذلك جرى التوقيع .
