بدأ ظهور الخدمة الاجتماعية الطبية في إنجلترا عام 1880م وسبب ظهورها في هذا المجال للعناية بالمرضى المصابين بأمراض عقلية لأنهم يحتاجون إلى الرعاية قبل خروجهم من المستشفى ، وتقدم لهم هذه الخدمة حتى وهم في بيوتهم حتى يتجنبوا انتقال وتكرار المرض ، وأصبح في إنجلترا حركة التطوع في خدمة ومساعدة المرضى وهذه الحركة ظهرت عام 1890م وتزعمها –تشارلزلوك - ، وبدأت ذلك في مستشفيات في نيويورك عام 1904م وتم طرح نظام جديد بأنهم يقوموا بإرسال بعض الممرضات والتي أطلق عليهم اسم (الزائرات) إلى منازل المرضى ليقوموا بتقديم النصح والإرشاد بكيفية التعامل مع المرض .
ومن الخطوات المهمة التي حولت هذه الخدمة من الدراسة الأكاديمية إلى الممارسة العملية كان ذلك عن طريق طلاب كلية الطب الذين قاموا بتجربة تدريبية وعملية في المؤسسات الاجتماعية وكان ذلك عام 1902م عندما طلبت جامعة بالتيمور من طلابها أن يكونوا متطوعين في المؤسسات الاجتماعية الخيرية لكي يكتسبوا الفهم والإدراك للقضايا الاجتماعية وآثارها ، وكذلك الاقتصادية وينظروا إلى ظروف المعيشة في حالة المرض ،وترتب على تلك الخبرات هو إنشاء الخدمة الاجتماعية عام 1905م في كثير من الجهات المختلفة بولايات أمريكا ، وأصبح الأخصائيون الاجتماعيون أعضاء وموظفين في المستشفيات العامة ، ويرجع ذلك الفضل الكبير في تطوير الخدمة الاجتماعية الطبية للطبيب ريتشارد كابوت وكان في مستشفى ماساشوش بمدينة بوسطن عام 1905م وفي نفس العام أنشأ قسم خاص في نفس المستشفى يعتنى بالخدمة الاجتماعية الطبية ، ولم يلبث أن مضى عشرون عاما على البداية إلا وتم إنشاء أكثر من 500 قسم للخدمة الاجتماعية الطبية في أمريكا ، وكان الأطباء في مستشفى ماشوست هم أول من لمسوا إلى الحاجة للأخصائي الاجتماعية لمساعدة المرضى بعد خروجهم من المستشفى .
وتم إنشاء الجمعية الأمريكية للأخصائيين الاجتماعين الطبيين عام 1918م وهدف هذه الجمعية هو أن ترفع المستوى التقني والفني للخدمة الاجتماعية المتصلة بشؤون الصحة والرعاية الصحية ، وبعد ذلك تم إنشاء الخدمة الاجتماعية الطبية في مصر .
